PHARMACOLOGY
Pharmacodynamics
Nimesulide is a non-steroidal anti-inflammatory drug that selectively inhibits cyclooxygenase-2 but it appears to exert its therapeutic effects through variety of other mechanisms as well like inhibition of platelet-activating factor[PAF], tumor necrosis factor-alpha[TNF-], protease histamine etc.
Pharmacokinetics
Oral absorption of Nimesulide also complete. Concomitant administration of food may decrease the rate but not the extent of absorption of Nimesulide. Plasma binding is high [approximately 99 ]. Nimesulide is extensively metabolized to several metabolites which are excreted mainly in the urine [about 70 ] and in feces [about 20].
USAGE
Nimesulide, being an NSAID exhibits anti-inflammatory analgesic properties. It is indicated as the second choice for the treatment of acute pain and primary dysmenorrhea
SPECIAL WARNINGS AND PRECAUTIONS FOR USE
Hepatic effects
Rarely Nimesulide has been reported to be associated with serious hepatic reactions, including very rare fatal causes. Patients who experience symptoms compatible with a hepatic injury during treatment with Nimosulide [ e.g. anorexia, nausea, vomiting, abdominal pain, fatigue, dark urine] or patients who develop abdominal liver function tests should have treatment discontinued. These patients should not be rechallenged with Nimesulide. Liver damage, in most cases reversible, has been reported following short exposure to the drug

CONTRADICTIONS
The use of Nimesulide is contraindicated in patient with hepatic impairment, children, active peptic ulcer disease, known hypersensitivity to the active substance, acetylsalicylic acid, or other prostaglandin synthetase inhibiting drugs
ADVERSE REACTIONS
Undesirable effects include gastrointestinal disorders, gastrointestinal bleeding, duodenal ulcers and perforation, gastric ulcer and perforation
SIDE-EFFECTS
At therapeutic dose Nimesulide is usually well tolerated. Occasional side-effects include heartburn, nausea, and epigastric pain. Skin rashes of allergic-type may rarely occur
Uncommon side effects include bleeding from stomach or bowel; duodenal or stomach ulcer and burst ulcers
DRUG INTERACTIONS;
Clinically important interactions involving drug metabolism have not been described with Nimesulide
DOSAGE AND ADMINISTRATION;
Usual oral dose in adults is one tablet twice daily for not more than 15 days
